*Required every 3 years*

 It is the student's responsibility to return PAGE 1 of the physical form to the activities office.  Please be sure to have your healthcare provider give you a copy of the first page.  Your healthcare provider should keep the full physical form in their medical file.

$20 Sports Physicals - Thursday, August 3 at Center for Specialty Care

All area high school athletes who need a sports physical for the 2017-2018 school year are invited to the Center for Specialty Care on Thursday, August 3 between 3:00 p.m. – 6:00 p.m. If this time does not work for you, just call the Center for Specialty Care at (507) 238-4949 to schedule an appointment.

The fee for sports physicals is $20. It gets better. Center for Specialty Care donates Half of your fee back to our athletic department.

ADMISSIONS TO HIGH SCHOOL SPORTING EVENTS & MUSICAL/PLAY

ALL Fairmont Elementary and High School students will receive FREE admission this year by presenting their ID badges at the pass gates of all home events (tournaments excluded).

Who receives an ID Badge? 

High School Students:  Any registered student taking at least one class at Fairmont High School.
 Elementary Students: The elementary school will follow the same procedure as the high school with issuing ID badges. Any home schooled, SJV, or SPL students who take at least one class at the public school will receive an ID badge. Ex: band or orchestra.

How does an elementary student receive an ID Badge?
Once elementary school starts, a school staff member will take care of photos for ID badges. For events before the elementary students receive their ID badges, students will be admitted free with their family at the pass gate.

Fairmont Junior and Senior High Schools offer a wide range of music, fine arts, and athletic activities for the student. Contact the high school office at 238-4411 for all questions relating to activity participation. Prior to the first practice, students must register each season on the appropriate activity registration dates and complete the following: sports physical (every 3 years), MSHSL contract (once a year), and Activity Registration with fee (each season).
